About Your Visit

What you should bring to your appointment:

  • Health Insurance Card/Information
  • Referral from your primary care doctor if required by your insurance
  • A list of all medications you are taking
  • A copy of your X Rays/CT scans (reports and actual films, NOT CD)
  • Copies of records from doctors related to your ear, nose and throat problems
  • Completed FORMS (see below)

In order to expedite the patient registration process, please download the registration forms below, print all four pages, fill it out and bring them with you on your first visit to our office. Thank you!
